I am with you on all of this, except that item #3 violates the first amendment. You'd probably have to allow for religious exemptions.
I just found out that me 31-year-old son is NOT vaccinated, and he lives with his severely-immunocompromised mother. Because of her disabilities, SHE shouldn't get the vaccine. But my own son could accidentally KILL her. I am appalled.
Meanwhile, my younger (27-year-old) son is ALSO unvaccinated, and I have NO idea why. I raised these guys right, and they aren't Republicans or Fox News fans... but they are derelict in their duty to America (and family.)
They are both adults, and must make their own decisions (which I must respect.) That said, I am horrified by their lack of attention to the horror that they do not seem to realize is stalking them with 1000X more virulence than the original Covid virus.
I don't know whether to be upset, sad, or pissed at their lack of concern for safety and community. (Probably all of the above.)
